 Kindergarten Information (2024-2025)

  • If you know of anyone who lives in Howard’s neighborhood and has a kindergarten or new student, let them know the time to enroll is now! Students must be enrolled by Friday, September 2nd to attend the first day of school. If your student attended Howard last year, you’re already enrolled for this year.

     

    Our office is open 9am-3pm daily for the remainder of the summer. Our office will be open at 8:00am when students return to school on September 4th. New (and returning) families are welcome to contact our secretary, Becky Lamb, at (541) 790-4900 or lamb_b@4j.lane.edu.

Kindergarten Enrollment Information

It’s time to start thinking about where to send your kindergarten age child to school next fall.  I would like to recommend Howard Elementary School.  Our school is strongly connected to the community and enjoys great support.  Students receive carefully planned instruction in all curriculum areas, including library, full-time counseling, physical education and music.  Parents are considered essential partners at school and are welcome to participate throughout the year.  Howard is an exceptional place for children and I invite you to come see the special experience Howard Elementary can provide for your child. Please start your visit in the office.

  • Enrollment form, completed with a minimum of one emergency contact other than parent(s).
  • Proof of birthdate –Mandatory
    • ORIGINAL Birth Certificate with embossed seal or on watermarked paper
    • Passport
    • Matricula Consular card
    • Oregon Health Plan Document (original, not copy)
  • Or TWO of the following
    • Hospital Record Form
    • Women’s Infant/ChildrenProgram ID card
    • Food Stamp ID card
    • Social Security ID information that accompanies the SSN card
  • Proof of address (i.e. EWEB bill, phone bill, rental agreement – MANDATORY)
  • Completed immunization record with a minimum requirement of the following:
    • 1 Diphtheria/Tetanus
    • 1 Polio
    • 1 Hepatitis B
    • 1 Hepatits A
    • 1 Varicella or date of disease (Chicken Pox)
    • 2 Mumps, Measles, Rubella combination
    • The following will be required to be up to date by February: 5 Diphtheria/Tetanus, 4 Polio, and 3 Hepatitis B
  • Physical Exam Form (MANDATORY)
  • Parent Health Evaluation form
  • Transportation Request form (MANDATORY – if living further than 1 mile from school)
